Equipment Sign-out
This form is used to document the sign-out of non-consumable simulation equipment by faculty. Non-consumable items include reusable materials such as task trainers, manikins, tablets, cameras, or other durable equipment that can be returned after use. Consumable items such as gloves, syringes, or simulation supplies that are used up during a session should not be requested through this form.
Only equipment specified and approved by the Department Head or a designated authority may be signed out. By completing this form, faculty acknowledge responsibility for the equipment during the loan period, agree to return it on time and in the same condition received (normal wear expected), and understand that late returns or repeated negligence may result in restrictions on future sign-outs or disciplinary actions as outlined in department policies.
